Problem – For nurses, the delivery of all aspects of the patient care requirements across the 24 hour period is a crucial aspect of their role (ref ). In the current climate of complex care within resource scarcity the risk of missed care is high (ref). For the nursing student this risk is magnified due, in part, to inexperience but also the lack of autonomy and confidence to make decisions (ref ). Students learn much of the skills required to make decisions and perform tasks in simulated university settings, only gaining the opportunity to put into practice some of the skills when the opportunity presents during clinical placement. Sometimes there is conflict between what the students understands to be the correct process and what the experienced nurse has adapted to become their standard practice (ref). Documentation is a legal requirement and evidence of care delivery, it is also a crucial aspect of communication for continuity of care (Demsash et al.,2023). Incomplete documentation may represent missed care or lead to potential missed care. Using the NSQHS Comprehensive Care Standard and Communicating for Safety not only guided the correct care but also gave this student the confidence to respectfully challenge behaviours that could potentially lead to missed care. Purpose – Care plans are the amalgamation of a comprehensive assessment to determine the 24-hour care that the nursing team are required to deliver (ref). Inaccurate or incomplete documentation is known to represent x% of all adverse events in the year 2021/2022. During a clinical placement the student was asked to sign off on the mornings’ care plan -before tasks had been completed. Appreciating that missed care represents the omission of part or all of required care to meet patient needs and contributes to poor patient satisfaction and adverse events (ref). The student was aware that the risk of not completing some tasks due to the priority of others was possible and that the communication of the incomplete was essential to the critical information being handed over to the next shift. The care plan acts as a document to not only communicate the care that is completed but the care that awaits completion. The intent of the Comprehensive Care standard is a patients preferred and required care is comprehensively planed and coordinated, the care plan allows for such coordination through the record of care completed each shift (ref). The Clinical Governance standard outlines the requirement for systems to be in place to support the delivery of comprehensive coordinated care (ref). Knowing this, the student argued that the correct procedure was to sign the care plan at the completion of the shift, thus ensuring incomplete tasks were safely communicated to the next shift.
Outcome – From a student perspective the concern felt for not completing tasks can feel overwhelming. Review of the NSQHS standards offers not only clear guidance on aspects of high-risk activities but also acts as a salient reminder that care is a 24 hour responsibility shared between a multidisciplinary team. Communicating for Safety acts as a safeguard to ensure that critical information is shared, and the Comprehensive Care Standard reminds us of the need to coordinate care through throughout assessment and communication on a shift by shift basis.
