Identification of Focus for Community Teaching (Topic Selection):

Adults are more vulnerable to cardiovascular diseases than other populations due to their susceptibility to risky behaviors such as cigarette smoking, obesity, alcoholism, physical inactivity, and diabetes. Rodgers et al. (2019) present age as a non-modifiable factor for cardiovascular diseases because it leads to hormonal imbalances and declined body functions. As a result, these factors render the population at risk of cardiovascular diseases (CVDs).

• Cardiovascular diseases are the leading causes of death in the United States
• About 659000 people die from heart disease annually in the country (Centers for Disease Control and Prevention, 2022)
• 1 in 4 people die from heart disease in the US

Teaching Plan Criteria

• The community teaching plan will include the following components;
• Establishing the program objectives
• Obtaining funding for the program
• Forming teams and partnerships
• Selecting target audience
• Deciding on convenient sites and locations to conduct educational sessions
• Implementating education sessions
• Evaluating the project
• Sustaining, revising, and updating depending on feedback from the previous phase.

Nursing Diagnosis:

Nurses will monitor vital signs before and after the plan implementation to assess its effectiveness in improving symptoms of cardiovascular diseases. Also, they will screen participants during the implementation phase of the project to obtain various parameters such as weight and heart rates.

Readiness for Learning:

The criteria for assessing participants’ emotional readiness will focus on their willingness to attend all class schedules, their levels of interactions with facilitators, and their overall curiosity and interest in acquiring knowledge of proper interventions for managing cardiovascular diseases. On the other hand, the approaches for evaluating participants’ experiential readiness will entail assessing the intrinsic motivation, locus of control, unconditional engagement, and their past experiences in learning.

As a facilitator, I will use this theory to create a conducive learning environment where learners can communicate effectively, air their concerns and opinions, and acquire knowledge of proper cardiovascular disease management interventions.

Goal:

This teaching plan is consistent with Healthy People 2020’s objectives of reducing coronary health disease deaths and reducing hospitalizations of older adults with heart failure as the major diagnosis (Healthy People 2020, n.d.). Equally, Healthy People 2020 presents healthy nutrition and physical activeness as profound strategies for reducing the prevalence and mortality rates of cardiovascular diseases.

Alma Ata’s Declaration of 1978 focuses on concerted interventions for increasing life expectancy by preventing preventable diseases. Develop Behavioral Objectives (Including Domains), Content, and Strategies/Methods:

The project implementation team will demonstrate creativity by creating visuals, including posters, PowerPoints, and charts. It is essential to note that creativity is a profound aspect of a health promotion plan because it enables people’s engagement in the learning processes. For example, the ability to document content into simple and engaging charts is an essential example of creativity in patient education programs.

1. Improving learners’ knowledge and awareness of healthy foods and self-care interventions. It is crucial to assess their knowledge by examining their familiarity with learned concepts.
2. Improving individual ability to abandon risky behaviors. I will conduct follow-up activities to assess participants’ ability to positively modify behaviors
3. Improving individual ability to complete moderate physical exercise. I will virtually interact with patients and home-based care providers to assess the participants’ ability to complete the recommended sessions for physical exercise.
4. Reducing the number of reported complications associated with CVDs. I will utilize the organizational benchmarks and records to compare the incidences of cardiovascular diseases before and after the interventions.

I will conduct a summative assessment to establish whether the project aligns with the strategic goals of improving the community’s knowledge and awareness of self-care interventions for managing cardiovascular diseases, including healthy diet plans, behavioral modification, and physical exercise.

Planned Evaluation of Lesson and Teacher (Process Evaluation):

Formative assessments, including daily monitoring of people attending the learning sessions and frequent collection of participants’ perceptions, feedback, and opinions on the project will form the basis of the process evaluation process. It is essential to note that process evaluation will enable the implementation team to identify areas of improvement and the underlying impediments to effective project implementation.

Barriers:

The potential barriers to the community teaching plan include insufficient funding for the exercise, a lack of standardized health education programs for preventing cardiovascular diseases, and unwilling human resources to implement the project due to poor remuneration. It is possible to address these problems by mobilizing stakeholders to obtain resource support, utilizing evidence-based practice to identify and implement the proven approaches for educating patients, and creating a sense of urgency to obtain employees’ buy-in. Therapeutic Communication

4.2 Communicate therapeutically with patients.

Therapeutic communication can be essential in increasing diagnosis accuracy, facilitating consensus decisions, and creating a positive environment for recovery (Sharma & Gupta, 2022). I will begin the presentation by introducing the topic, providing a brief overview of the statistics for the topic, and stating the objectives of the project. In terms of enhancing active listening, I will allow participants to ask questions and provide an in-depth response to their queries. Thirdly, I will combine verbal and nonverbal communication strategies such as using demonstration, asking questions, proper posture and eye contact, tone and utilizing examples and statistics to appeal to credibility.
