This assignment will demonstrate your ability to provide age-appropriate anticipatory guidance while recognizing the need to refer patients that are outside of the scope of practice of the family nurse practitioner. This will be demonstrated by completing a SOAP note based on a patient Anna Jones seen in Unit 6 in the VR platform.

- It is an important reference document that provides concise information about a patient’s history and exam findings at the time of patient visit.

Unit 2
- Case: Angela Atwater – Hypertension Stage 2, and Overweight
Scenario Description:
Ms. A presents with complaints of headache and general fatigue. She also reports that she has had periodic nosebleeds. She reported that she has been taking many medications for her hypertension in the past, but stopped taking them because of the side effects. She could not recall the names of the medications. Prescribed one year ago and currently taking 100 mg/day atenolol and 12.5 mg/day hydrochlorothiazide (HCTZ), which she admits to taking irregularly because “… they bother me, and I forget to renew my prescription.” Despite this antihypertensive regimen, her blood pressure remains elevated, ranging from 150 to 155/110 to 114 mm Hg. In addition, Ms. A admits that she has found it difficult to exercise, stop smoking, and change her eating habits. She has not seen her primary care doctor in a year. Smokes 15 cigarettes a day for 10 years.

Unit 4
- James Hanson: Right Great Toe Pain
Scenario Description:
Male 47-year-old African American Male. Patient came to the primary care clinic today complaining about left great toe pain. Woke the last 2 nights in pain. Left toe red and swollen at the joint. He describes the pain as throbbing and feels like his toe is pulsating. Reports he has had periodic pain in the joint before but nothing as bad as what he is having now. He reports to have taken aspirin for the pain the last 2 nights, yet nothing seems to help. He did put an ice pack over his toe last night and that seemed to help a little with the pain. He denies any trauma or injury to the toe or left foot. He did say that he had a “few too many” beers with the guy’s last night. Today his left toe hurts so much that he can’t even stand to wear a shoe or put weight on his left foot. He just cannot get comfortable and does not want to spend another sleepless night. He has never been seen for this issue before.

- Anna Jones: Low back pain:
Scenario Description:
Patient is married and lives with her husband. She works full time as a nursing assistant and her husband is a full-time mechanic. Patient complains of low dull and throbbing back pain irradiating down to left buttock to lower extremities (foot area) after lifting and turning a patient in the bed at work three days ago. Patient reports she felt “pop” and immediately started developing back/buttock pain which seems to be worse especially when sitting, no better despite resting and taking ibuprofen every 8 hours for the pain. Reports that the pain seems to also be getting worse, stinging and tingling radiating down her left leg into her left calf. Having trouble walking and climbing stairs; cannot get comfortable.
